Throw Out Bearing Travel

Had my throw out bearing/clips come off last trip and trying to figure out why. Just rebuilt the trans and put a new throw out bearing on as well. It’s a Mendeola 2D with a Subaru 2.5, Kennedy Stage 3 clutch, and CNC slave cylinder (not sure of the bore but 1” of scope). Not sure if I adjusted the the release bearing too close or even touching the pressure plate, too much throw, or ???. Springs were bent laying on the bottom of the bell housing and the inner plastic bushing was cracked. Just wondering if anyone has experience with this. 

 
Simple issue could be that the clips did not get attached all the way behind the fork/arms, and the bearing simply fell off.

If the clips were properly attached and it still came off the throw out arms, the common issue is throwing the pedal/slave cylinder too far into the pressure plate and the throw out bearing interferes with the fingers of the pressure plate, ripping the bearing off the throw out arms.

The clutch pedal only needs to travel far enough to disengage the clutch. If the pedal can travel more than one inch past the point where the clutch is open, you risk pushing the throw out bearing too far into the clutch fingers.

Use a clutch pedal stop to adjust how far the pedal needs to throw only to open the clutch.
